| Does my team need permission to travel? The Dog Days of Summer’s sanctioning body, OSA, requires a "travel permit" form for out of state teams. |

| What documents will I need to register when my team is accepted? 1. League Approved Roster 2. Guest Player Roster: must be generated through Ohio Soccer Association and send us the approved copy 3. Travel Permit (non OSA teams only) 4. Liability waiver 5. Concussion Awareness certificate if your team is from any state other than Ohio 6. Registration Checklist All forms must be submitted through the tournament website. Login with your Team ID and upload forms. |
